Walking Each Other Home is an integrative palliative care and end-of-life navigation practice founded by Dr. Julie Johns, a board-certified emergency medicine physician with over 20 years of experience in acute care and a passion for conscious living and dying.
Dr. Johns knows how difficult it can be for caregivers and patients to navigate the complexities of the healthcare system. She is here to help you better understand each step of your illness and, if needed, your dying journey. She will also help you communicate to your healthcare team your goals of care. She can help you navigate difficult conversations within your family, whether you are the patient or the caregiver, with the utmost attention to autonomy and dignity.
This is not a second opinion service. Rather, Dr. Johns offers a highly-experienced, conscious, compassionate presence to help you navigate the complex healthcare system and to be your advocate. Often this involves meeting with a patient and their family to assure alignment with and understanding of the patient’s unique situation. Together we can create a communication plan for the patient, family, caregivers to take with them to appointments. This may even involve Dr. Johns accompanying the patient and family to future appointments, in person or virtually, in order to translate complex medical information through the lens of your unique situation and goals of care.
