Our Packages
Please note, these pricings are just the start of our services. However we understand that end of life can come more quickly than anticipated, so these are guidelines for what you can expect. We will custom tailor our plans for you and your loved ones based off of your needs.
This includes the initial 90-120 minute in depth consultation with patient and primary caregiver and Dr. Johns as well as an additional 120 minutes of follow up advocacy and care coordination (may include virtual attendance at medical appointments, calls to referral sources on your behalf, patient and family care conferences to ensure you understand all options being presented to you)
Additional physician-led advocacy and coordination services starting at $300/hour
